Comparative Study of Benign Gastric Ulcer with Ulcerating Stomach Cancer

Abstract
Although the combined use of laboratory and diagnostic studies including roentgenography, cy
tology, gastroscopy and gastric juice analys is affords a diagnostic accuracy of approximately 90%,
gross finding of lesions at operating table is also very importat to anticipate the benignancy or
the malignancy of the lesions.
This clinical review is a comparative study of benign gastric ulcer with ulcerating stomach cancer
for the size and location of lesion in stomach. During the past 11 years from January 1, 1959 to
December 30, 1969, 744 cases of gastrectomy were done at Medical Ceneter, Yonsei University
College of Medicine, either for benign gastric ulcer of ulcerating stomach cancer. Of the 744 cases
of gastrectomy, all malignant gastric ulcers(446 cases) were compared with 114 cases of 243 benign
gastrici ulcer whose clinical and pathological records are available. The results are follows:
1. THe ratio of benign ulcers to malignant ulcers compared is 1:3.9.
2. The peak age incidence of both gastric ulcer and ulcerating carcinoma is between 50 and 59
years, each accounting 172 cases(35.3%) in ulcerating gastric carcinomas and 38 cases(33.3%)in
benign gastric ulcers.
3. Male to female ratio was 2.3 : 1 in ulcerating gastric carcinomas and 7.5:1 in benign gastric
ulcers.
4. Clinical symptoms in order of frequency were as follows: Epigastric pain and discomfort 258
cases(27.2%), Nausea and vomiting 30 cases(26.3%) and Hematemesis and melena 27 cases(23.7%)
in benign gastric ulcers.
5. The most of malignant gastric ulcer patients(72.5%) had the duration of symptoms less than
one year and more than half of the benign ulcer cases(58.9%) had more than three years of
duration.
6. The diameter of gastric ulcer was larger than 3§¯ in 92.8% of malignant ulcer and smaller
than 5§¯ in all cases of benign gastric ulcer.
7. The ulcers located in antum of stomach or greater curvature show high incidence of
malignancy and most of benign gastric ulcers were located in pylorus and/or lesser curvature site.
